Community Memorial Foundation’s vision of a healthy community is one in which all persons – regardless of race, gender, sexual orientation, creed, age, national origin, means or background – have access to coordinated health and human services necessary for a healthy life.

Mission

To measurably improve the health of people who live and work in the western suburbs of Chicago.

The Foundation works in partnership with non-profit organizations to develop, implement, and sustain creative programs and services that have a measurable impact on the health and well being of our target communities.
